Problems with Signature widget not saving signature

I am having problems with the signature widget.

I can sign my signature in the image box, and view the encoded URL in a text area:

but when I save the record and re-open it the image has disappeared. And it appears the sometimes the URL is not saved at all:

or else the saved URL appears to be incomplete:

Has anyone else encountered any similar issues?
