How to create a widget out of this code

Hi Mendix Community I have the following code I found on this link  https://jsfiddle.net/2vwcd3a8/  and I would like to be assisted with package this into a widget for Mendix 8.18.2 Am looking to use this widget on the hybrid app and the web/desktop profile. Your assistance is highly appreciated, Thanks Manar
